Expert interprets family cluster cases in Tianjin

Chen Wei, deputy chief physician of the Tianjin Center for Disease Control and Prevention, introduced the family cluster outbreaks in Tianjin, at a press conference on the prevention and control of the novel coronavirus pneumonia (NCP) on the afternoon of February 17. As of February 16, Tianjin had reported 26 family cluster epidemics, among which, there were 4 related to the passenger transport section of bullet trains, 9 involving the Baodi Department Store and 13 other kinds of family cluster epidemics, involving 75 cases, which accounted for 60% of the total confirmed cases in the city.

ThefamilyclusterepidemicinTianjinisfeaturedbythefollowingcharacteristics:

First, compared with the cluster epidemic suffered by the collective unit, the family cluster epidemic has a fast spread due to the frequent close contact among family members. It is found that when the first case of a family shows symptoms, the other infected family members will develop the disease on average 3 days later, while the average interval between the onset of the second case and the first case is 7 days in the collective unit.

Second, gathering serves as a major means of transmission among family members. Of the 26 family cluster epidemics, 10 (38percent) were caused by having get-togethers.

Third, a high secondary attack rate is recorded. 50 percent of the 26 family cluster epidemics havetwo or more second-generation cases, with the highest number of the cases reaching 5. And there were 4 second-generation cases, on average, in families with multiple gatherings.

On preventing the family cluster epidemics, here are some suggestions from Chen:

(1) Family members of an NCP case, as close contacts, need to go to the central quarantine area in the district, where single-room isolation can be ensured and transmission among family members is to be effectively blocked.

(2)Those under home-based quarantine after epidemiological investigation shall:

1. conduct single-room quarantine;
  2. reduce contact with family members. If the contact is necessary, wear a mask and keep a distance of more than 1 meter. During the quarantine, the masks and tissue and other wastes shall be thrown in special garbage bags and uniformly disposed by related administrative personnel.

3. have dinner separately with family members to avoid sharing dining utensils and other items; use a separate toilet, otherwise, it is necessary to stagger the toilet time anddisinfect the toilet in time after use;
  4. keep their family membersaway from the isolation room, where items shall be disinfected and then washed before they are touched.
  5. open windows to help with ventilation at least twice a day, 30 minutes each.
  6. clean and wipe the items down with clean water every day. Disinfect and clean the door handles and the surface of the toilet according to the instructions of community health personnel.

7. monitor their health conditions regularly as required during medical observation; contact with the person in charge of community medical observation and get timely treatment in case of fever, cough and other symptoms.

The headquarters of epidemic prevention and control in the districts of the city have set up service teams to provide life, medical and other services and compassionate care for the families of those who are under home-based quarantine, in ways that guarantee basic living needs as well as health and safety of the people.
(3)Precautions for families

1. Avoid gatherings with families, friends and neighbors, and group meetings amid the outbreak.
  2. Avoid going to densely populated public places of poor ventilation.

3. Wear a mask when going out. Wear a conventional medical mask when going out to crowded public places and taking public transportation, and a surgical mask is needed when going for medical treatment, including visiting a fever clinic.
  4. Open windows to help with ventilation at least twice a day, 30 minutes each.
  5. keep your hands clean. Reduce touching objects and equipment in public places.
  6. Keep good hygiene and health habits. wash hands, disinfect and clean frequently, and make sure that food is well cooked. Do not share towels and cups with family members, and keep home and tableware clean.
  7. Monitor health conditions of the whole family and measure body temperature when feel like having a fever. Prepare a thermometer, face masks and domestic disinfection and other supplies at home. Throw discarded masks in special dustbins.
8. Wear a mask and visit the fever clinic of a nearby hospital in case of suspicious symptoms like fever, cough, pharyngalgia and feebleness. Try to avoid taking subway, bus and other transportation. Tell the doctor your travel or living history to epidemic areas, and who you have met since you were infected. Cooperate with the health departments on relevant investigation.
